This site induction has been developed to observe safety practices for COVID-19 in accordance with the New South Wales Government requirements.

The induction is to be undertaken by contractors, suppliers and any other persons entering the site.

Every person must complete an individual sign-in every day.

> Have you received one dose of a COVID-19 vaccine and, if less than 3 weeks since that vaccine, a negative COVID-19 test in the last 72 hours.

> Do you meet a Government exemption for individuals with medical or other conditions that make vaccination unsuitable.

If YES, please proceed to Question 2.

If No please do not enter this work site. Contact the site supervisor and advise that you cannot undertake the work and return to your place of residence.

Have you been in contact with anyone who has returned a positive COVID-19 test, is a close or casual contact of a person with a positive COVID-19 test?*

If NO, please proceed to Question 3.

If YES:
 
Please do not enter the work site. Contact the site supervisor and advise that you cannot undertake the work then return to your place of residence.

Question 3

Do you feel well today and have no cold or flu symptoms or other symptoms of COVID-19?*
 Yes
 No

If YES, please proceed to Question 4.

If NO:
 
Please do not enter the work site. Contact the site Supervisor advising that you cannot undertake the work today and please go and get a COVID-19 test with a negative result before returning to this work site.
